Under the Privacy Act of 1974, Congressman Schiff’s office requires written authorization to assist you. To get started, please complete either the USCIS Privacy Release Form or the Casework Authorization Form as applicable (see below for details), and submit the completed forms by mail or fax to Congressman Schiff’s District Office in Burbank:
